Reminder
J-Term Students: Act by noon, Dec. 15

Dear New and Returning Students:

Welcome to all of our new students, and welcome back to everyone who is returning. This message outlines requirements you must meet related to the COVID-19 vaccine.

All students who live, learn, or work in person at the University must be fully vaccinated for COVID-19 before coming to the Grounds. A detailed set of FAQs is available to answer questions you may have about this requirement.

Students may seek a medical or religious exemption to the vaccination requirement. Links to the exemption request forms are available on the Student Health and Wellness website. Any student receiving such an exemption will be subject to pre-arrival testing, weekly prevalence testing, and likely other public health measures.

Students who are not vaccinated and do not qualify for an exemption are not permitted to come to Grounds, meaning they cannot move into a residence hall, attend class, use University services and resources, or otherwise be present on Grounds.

Please plan to submit your proof of vaccination or exemption form to the HealthyHoos patient portal (using NetBadge) as soon as possible, but no later than January 5, 2022. Students who are still in the process of being vaccinated should submit their documentation as close to January 5 as possible, and are not permitted on Grounds for any reason until they have done so. Failure to submit this documentation could cause delays or disruptions to your program start date.

J-Term Students, Please Note
If you are planning to attend a J-term course beginning January 3, 2022, you must submit your proof of vaccination or exemption form by noon on Wednesday, December 15, 2021. Failure to do so will result in disenrollment on Thursday, December 16, 2021, from any J-term courses. Please do not risk disenrollment.

Additional Requirements: New Students
If you are a new student, you may need to complete additional requirements, such as the Pre-Entrance Health Form and the Health Insurance Requirement.

Health and wellness are foundational to being a successful student and participating in the University community. Thank you for prioritizing this requirement, and we look forward to your upcoming arrival.

Sincerely,

Julie I. Caruccio
Interim Dean of Students
