Approval Request for Use of Alcoholic Beverages

In general, alcoholic beverages may not be served at any University function or event held on property where underage University students will be attending. However, requests for exceptions may be submitted via this form.

Complete this form if:

  • Your event will be held on University property (regardless of who is in attendance); or
  • You are a University department hosting a function (not on University property) in which students are expected to attend.

This form must be submitted at least five (5) business days prior to the proposed event. Approvals are issued on an event-by-event basis.

Funds for Student Programs

The Office of the Vice President and Chief Student Affairs Officer funds a variety of (mostly on-Grounds) programs that promote citizen leadership in one of these five development areas:

  • Social
  • Educational/academic
  • Cultural
  • Service
  • Other

Program funding is event-specific, and may only be used to pay pre-approved expenses (funds may not be used for ongoing operational expenses).

Request for Student and/or Parent Data

UVA-affiliated departments, organizations, or individuals seeking to access student or parent data of any kind (beyond legitimate educational need) must submit this form for approval in accordance with University guidelines. Fulfilling requests will take three (3) business days on average from the date of approval.

The weekly Connections newsletter reaches all UVA undergraduates as well as students in the Graduate School of Arts & Sciences. Submit your entry by noon on Monday for inclusion in the following Wednesday's edition.

Arrest Self-Disclosure Form

Each year, new and returning students are asked as part of their Annual Student Update Form, whether or not they have been arrested or convicted of a crime. Students must report this information if they have not previously provided it to the University.
